Practice closures due to staff training

The Practice will be closed on the following date due to staff training:

Thursday 13 October from 12.00 noon. The Practice will re open on Friday 14 October at the usual time.

Practice closures due to Bank Holiday

The Practice will be closed on the following dates due to Bank Holidays:

Monday 29 August 2016, the Practice will re-open on Tuesday 30 August.

Monday 26 December 2016 and Tuesday 27 December 2016, the Practice will re-open on Wednesday 28 December 2016.

Monday 2 January 2017, the Practice will re-open on Tuesday 3 January 2017.

View your Medical Records Online

The Hedon Group Practice offer patients the opportunity to view areas of their medical records online using the system Patient Access.

If you would like to view your medical record, you will need to read our Information Leaflet (FAQ), and request a consent form at Reception. Please bring photo ID with you when submitting your consent form (e.g. a passport, driving licence).

Your medical record will be available for online viewing no later than 28 days from your request. 

Please go to reception for more information and to request the Patient Access to Medical Records Consent form.

Friends and Family Test (FFT)

The FFT is a feedback tool which offers patients of NHS-funded services the opportunity to provide feedback about the care and treatment they have received.

The FFT tool combines a simple question, asking patients how likely they would be to recommend the service they have received to their friends and family; with at least one complementary follow-up question to enable patients to provide further detail about their experience.

To provide your feedback, please click on the "Friends and Family Test" link on the right navigation pane under the "Have your say" section OR ask at reception for a form.

On Line Appointment Booking now available

We now have a range of appointments that can be booked (or cancelled) on line. Please complete the necessary documentation available from reception.

To reduce the number of missed appointments we are now sending patients a text message containing their appointment details when a booking is made.

A further message will also be sent 48 hours before the appointment is due as a reminder.

Pneumonia vaccination

All patients aged over 65 are eligible for a pneumonia vaccination. This is a single injection that provides lifelong protection. If you will be aged 65 or over on 31st March 2016 and have not been vaccinated, please book via reception.
