Dispenser

We are seeking to recruit an enthusiastic and self-motivated NVQ Level 2 or equivalent Dispenser to join our friendly team. In return we offer a friendly and welcoming working environment where you will be supported to develop your own skills, knowledge and experience. The main duties will include processing and dispensing of prescriptions. Providing an efficient and friendly service for our dispensing patients. In this position you will work alongside our team of dispensers and dispensary management team.

Up to 37.5 hours to be worked across Monday - Saturday.

Care Coordinator

Job summary

As one of our Care Co-ordinators, you will be responsible for supporting the practice in delivering excellent patient care by effectively co-ordinating different elements of care from multi-disciplinary teams within and outside the practice.

We are currently seeking to recruit an additional Care Co-ordinator to join our expanding Proactive Care Team. Working alongside our existing Care Coordinator team, Social Prescribing Link Workers and Health & Wellbeing Coaches as well as our GPs and clinical teams, you will develop packages of care suited to the needs of individual patients.

Main duties of the job

Our successful candidate will have a busy and varied role including service co-ordination, active care planning for patients and providing direct patient support. You will co-ordinate multidisciplinary team meetings, liaise with clinical and non-clinical teams within Holderness Health and across our partner agencies.

Acting as a key point of contact for patients, carers and healthcare professionals, you will ensure that an individual’s healthcare needs are addressed in a joined-up way, ensuring that they receive the right care from the right professional at the right time.

You will have excellent communication and negotiation skills and be confident managing your own workload. Our ideal candidate will have experience of working in a health or social care setting and a deep understanding of patient care needs, but we are also interested in hearing from people from a wide range of backgrounds who can demonstrate sound administrative skills and experience of delivering high quality customer or patient care.

Advanced Clinical Practitioner

Part Time – Full Time

Band 8a equivalent, depending upon experience

We are seeking to recruit an ACP with an independent prescribing qualification and who is looking for their next career development opportunity. Experience of managing acute illness would be very welcome but we are keen to hear from all ACPs who have relevant skills transferable to Primary Care.

We are the largest GP practice in the East Riding and are seeking an Advanced Clinical Practitioner to join our multi-disciplinary team on either a full time or part-time basis.

We have the scale, diversity and breadth of skills within our large team to help you to develop your career, pursue areas of interest and take advantage of the opportunities we offer.  With truly local control we intend to keep the best of traditional patient centred Primary Care with all the benefits of a larger organisation.  Most importantly for you, we have extensive experience in providing a supportive environment for ACP’S as part of our diverse clinical team.  

About this Role

To provide care for the presenting patient and demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within general practice. Diagnose and treat patients who present with minor ailments and minor injuries, supported by GPs, advanced nurse practitioners and other members of the nursing team. Attend urgent home visits. The advanced clinical practitioner will work collaboratively with the general practice team to meet the needs of patients and support the delivery of policy and procedures. They will have the ability to prioritise and make any necessary referrals for investigations in the appropriate manner and be flexible and responsive to patient needs.

About You

You will have a relevant qualification at Degree Level or equivalent as well as having up to date membership of a relevant professional register.

Post qualification experience is essential to this role.

Experience of working in primary care would be welcome but we are also interested in hearing from candidates with appropriate experience in other healthcare settings.
